In technical terms Cialis exerts its action by effecting two enzymes, Cyclic guanosine monophosphate (cGMP) and phosphodiesterase type 5 (PDE5) that play important roles in erectile function. Sexual stimulation triggers the local release of nitric oxide (NO) from nerves and endothelial cells in the penis. NO stimulates an enzyme that catalyzes formation of cGMP from the nucleotide guanosine 5-triphospate (GTP) in the penile blood vessels and corpus cavernosum walls.
Administration of CIALIS to patients who are using any form of organic nitrate, either regularly and/or intermittently, is contraindicated.
The cGMP causes vasodilation of arteries, allowing the influx of blood into the corpus cavernosum. Increased blood flow into the penis causes the erectile tissue to expand. When the erectile tissue has expanded sufficiently, the veins draining the corpus cavernosum become compressed. This decreases the outflow of blood from the penis, and the blood remains in the corpus cavernosum to keep the penis erect.
Cialis Contraindications
Administration of CIALIS to patients who are using any form of organic nitrate, either regularly and/or intermittently, is contraindicated. In clinical pharmacology studies, CIALIS was shown to potentiate the hypotensive effect of nitrates. This is thought to result from the combined effects of nitrates and CIALIS on the nitric oxide/cGMP pathway
Alpha Blockers — Administration of CIALIS to patients taking any alpha-adrenergic antagonist other than 0.4 mg once-daily tamsulosin is contraindicated. In a drug-drug interaction study, when tadalafil 20 mg was administered to healthy subjects taking doxazosin (8 mg daily), there was a significant augmentation of the blood-pressure-lowering effect of doxazosin
CIALIS is contraindicated for patients with a known hypersensitivity to tadalafil or any component of the tablet.

Cialis Overdose
Single doses up to 500 mg have been given to healthy subjects, and multiple daily doses up to 100 mg have been given to patients. Adverse events were similar to those seen at lower doses. In cases of overdose, standard supportive measures should be adopted as required. Hemodialysis contributes negligibly to tadalafil elimination.
